Telehealth Consent

Effective Date: March 30, 2026  ·  Last Updated: March 30, 2026

DRAFT — FOR ATTORNEY REVIEW. This document is a working draft and has not been reviewed by legal counsel. It is not yet effective or enforceable. Do not publish without attorney sign-off.

This page provides an overview of what patients consent to when receiving healthcare services through PeptideVIP. It is intended as a plain-language summary to help you understand the nature of telehealth care before you begin the intake process.

The formal, binding telehealth consent is signed electronically during your intake process in our patient portal. This page does not replace that consent — it supplements it.

PeptideVIP is NOT an emergency service. If you are experiencing a medical emergency, call 911 or go to your nearest emergency room immediately.

What Is Telehealth?

Telehealth is the delivery of healthcare services using electronic information and communication technologies. Rather than visiting a provider in person, you interact with your prescribing physician or licensed provider through digital tools — including secure online health assessments, encrypted messaging, and your patient portal.

PeptideVIP uses telehealth to provide access to physician-prescribed peptide therapy to patients across all 50 states. In states where our physician holds a direct license, care is provided by that physician. In other states, your care is provided by a licensed healthcare provider within a clinical network, under physician oversight.

Benefits and Limitations of Telehealth

Benefits

  • Convenient access to physician care from home
  • No travel, waiting rooms, or in-person visits required
  • Available to patients in all 50 states
  • Asynchronous — complete your assessment on your own schedule
  • Direct provider messaging for protocol questions
  • HIPAA-compliant, secure patient portal

Risks & Limitations

  • No physical examination — remote assessment only
  • Technology failures (internet outages, platform issues) may cause delays
  • Not appropriate for emergencies or acute illnesses
  • Quality of care depends on accuracy of information you provide
  • May not be suitable for complex or high-acuity conditions
  • Not a substitute for primary care or specialty care

Patient Responsibilities

As a patient of PeptideVIP, you play an essential role in the safety and effectiveness of your telehealth care. By proceeding with our services, you agree to the following responsibilities:

  1. 1
Provide accurate health information. Complete all intake forms and health assessments honestly and thoroughly. Include all current medications, supplements, medical conditions, allergies, and relevant health history. Providing false or incomplete information may result in an inappropriate prescription and may cause harm.
  2. 2
Follow your provider's instructions. Adhere to the dosing protocol, administration instructions, and storage requirements provided by your prescribing physician. If you have questions, contact your provider before making changes.
  3. 3
Seek emergency care when needed. If you experience a medical emergency or acute symptoms that require immediate attention, call 911 or go to your nearest emergency room. Do not attempt to reach PeptideVIP for emergency clinical guidance.
  4. 4
Report changes in your health. Notify your provider through the patient portal if you experience side effects, changes in your health status, new medications, or any other changes that may affect your treatment protocol.
  5. 5
Maintain a private environment. When participating in any real-time telehealth consultation (video or voice), ensure you are in a private setting where your health information cannot be overheard by others.
  6. 6
Maintain primary care. PeptideVIP does not serve as your primary care provider. We strongly encourage all patients to maintain an ongoing relationship with a primary care physician for general health monitoring and non-peptide-related care needs.

Prescribing and Pharmacy

When your prescribing provider determines that peptide therapy is clinically appropriate for your individual situation, a prescription will be issued through your patient portal. Your prescription is then transmitted to a licensed 503a compounding pharmacy for preparation and shipment.

Key facts about your medications:

  • Physician-prescribed: Every medication is prescribed by a licensed physician or provider following a clinical review of your health assessment. No medication is dispensed without a valid prescription.
  • 503a Compounding Pharmacies: Medications are compounded by licensed 503a pharmacies, which prepare medications for individual patients pursuant to a valid prescription, in compliance with applicable FDA and state regulations.
  • Not FDA-approved products: Compounded medications are not FDA-approved drug products. They are prepared based on your individual prescription by a licensed compounding pharmacy. Your provider will discuss the evidence base and rationale for your specific protocol.
  • Nationwide shipping: Your medications ship directly from the compounding pharmacy to your door, typically within 2–3 business days of prescription processing.

Privacy and Security

Your health information is Protected Health Information (PHI) under HIPAA. PeptideVIP takes your privacy seriously:

  • HIPAA-compliant platform: All patient records, clinical notes, and communications are managed through PracticeBetter, a HIPAA-compliant electronic medical records platform. A Business Associate Agreement (BAA) is in place.
  • Encrypted communications: Data transmitted between you and our systems is encrypted using Transport Layer Security (TLS). Health information is encrypted at rest within our patient portal.
  • No data selling: We never sell, rent, or share your personal or health information for marketing or advertising purposes.
  • Access controls: Access to your PHI is limited to your care team and authorized personnel on a need-to-know basis.

For full details, please review our Privacy Policy.

Right to Withdraw Consent

Your participation in telehealth care with PeptideVIP is entirely voluntary. You have the right to withdraw your consent to telehealth services at any time, without penalty or effect on your ability to seek care through other means.

To withdraw consent or discontinue services, contact us at hello@peptidevip.com. Withdrawal of consent will not affect any care you have already received, but will prevent future prescriptions from being issued through our telehealth platform.

Please note that if you have medications in transit at the time of withdrawal, those medications have already been dispensed pursuant to a valid prescription and cannot be returned.

Formal Consent Is Signed During Intake

The formal, legally binding telehealth consent is signed electronically during your intake process in our patient portal (PracticeBetter). This page is an informational summary only — it does not constitute a consent agreement. You will be asked to review and sign the full consent document before receiving any clinical services.
